"A Symbol of Tradition and Unity" Registration Co Chairs All Registration Forms and Fees must be turned in to the Committee Member from your area by November 1st. If your area is not listed,please contact the Registration Chair. RegistrationForm RegistrationForm Registration Information: 1) Complete the Application and submit it to your local KCYL President with your check made out to KCYLNA (Individual Unit Contact information attached with this application) no later than November 1st, 2005. 2) Applications will be processed on a first come first serve basis. 3) Due to the limited number of total participants, there is a “Limit” for each KCYL Unit (Also attached to this letter) 4) Once you have submitted your application, you will be informed if your application has been accepted or if you are on the waiting list. 5) If accepted you may purchase your Airline Tickets and inform the individual you turned in the application to off your flight information. 6) If you are on the waiting list, you will be informed by November 19th if your application has been accepted or not. 7) At this time you may purchase your airline tickets if you have been accepted.
*KCYLNA Youth Summit 2005, participant distribution according to Local KCYL Units, as decided at the KCYLNA National Council Meeting in Chicago on September 17, 2005. The amount of space provided at the retreat center is minimal, it is asked that you return the packet as soon as possible. **Each participant who wants to take part in the KCYLNA Youth Summit 2005 should turn the entire registration packet to the respective person from their unit that is listed below. It is asked that you address the envelope with “KCYLNA SUMMIT 2005” as the first line. Transportation will be provided to and from the convention from O'Hare International Airport on Monday/Tuesday till 11:00 AM and Friday. Please indicate your arrival time on the registration form.
